Why do candidates fail the "biggest failure" question even when the story is true?

Common traps
- choosing a fake failure ('I'm a perfectionist') which reads as evasive
- blaming teammates/circumstances, signaling low accountability
- picking a disqualifying failure (cheated, missed a deadline that cost real money with no recovery)
- no lesson or, worse, repeating the same mistake
- rambling without STAR structure so the point is lost. The interviewer wants ownership + growth, delivered crisply in ~90 seconds.
